Dave (some external party) introduces them.  E.g. see the Powerpoint

starting on page 54.  If you're asking what I think you're
asking it's a pretty insightful question.  One that I think
MarkM would be delighted to answer if he had the time.  He
certainly delighted clarifying this aspect of Horton in the
talks we gave at Google and at Usenix HotSec.

At a high level the introduction is much like introducing
key pairs in a web of trust cryptographic system.

Nothing more specific than "introduction" that I know of.
